1. Take 101 to the San Antonio Ave. (West) exit
2. Go west on San Antonio Ave. approx. 2 miles and turn RIGHT on to El Camino Real
3. Go approx. 1.5 miles and turn RIGHT on to Ventura Ave. (between Chevron gas station & Hollywood Video)
4. Go RIGHT at the first stop sign on to Ventura Court
5. Follow Ventura Court into school parking lot
6. The Provider Connection is in Room 13 (see school map below)
   
   

1. Take the Page Mill Rd. East exit.
2. After 2 miles, go RIGHT on El Camino Real
3. Go almost 1 mile and turn LEFT on Ventura Ave. (in between Chevron station & Hollywood video).
4. Go RIGHT at 1st stop sign on Ventura Court.
5. Follow Ventura Court into school parking lot
6. The Provider Connection is in Room 13 (see school map below)
